New York program
Health coverage for an adult caregiver (Medicaid or the Essential Plan)
Medicaid and the Essential Plan offer health coverage for eligible adults caring for a child.
What it is
Medicaid and the Essential Plan offer health coverage for eligible adults caring for a child.
Your child's Medicaid does not cover your own medical care. NY State of Health can check both Medicaid and the Essential Plan for you. A recent pay cut may change which program fits.
Eligibility rules
- The 2026 parent Medicaid limits include $3,142 monthly for three people and $3,795 for four, including the income allowance.
- From July 1, 2026, the Essential Plan serves eligible adults ages 19–64 up to 200% of poverty. That is about $5,500 monthly for four.
- The agency reviews current monthly income and the adult's own eligibility conditions.
What you get
- Medicaid has no monthly premium.
- The Essential Plan has a $0 premium and low out-of-pocket costs.
What the help includes
- The Essential Plan provides comprehensive coverage but can have cost sharing.
- A parent and child can receive different coverage programs on the same family application.
If you decide to apply
- Ask a NY State of Health assister to include your own coverage needs in the family application.
- Have current household income, identity, New York address and any coverage-loss notice ready.

After you ask
- The usual Medicaid decision standard is 45 days. An Essential Plan enrollment notice gives that plan's start date.
- Above the Essential Plan limit, NY State of Health can compare Marketplace coverage and any available premium tax credit.
Good to know
The Essential Plan limit dropped from 250% to 200% of poverty on July 1, 2026. Earlier descriptions can show the old limit.
Other details
- A coverage change also means checking your own doctors and medicines against the new plan.
- An adult’s age, immigration category and access to employer coverage matter as well as income. The enrollment assister can confirm the current Essential Plan rules and compare any replacement Silver Enhanced plan’s premium, deductible and copays. For any future Medicaid work-reporting notice, the assister can check the requirements and caregiver exemptions that apply to that adult.

“Could I qualify for Medicaid or the Essential Plan after our income change? What costs or doctor changes would come with it, and could you help me compare and apply if it fits?”
Why I’m asking: I need to keep my own health care covered while caring for my child.

Who does what
The three parts, side by side. The agency decides; nobody on this page does.
You
Apply for yourself on the same application, report this month's income, and report the month work stops.
Your social worker
The social worker or enrollment assister reminds NY State of Health that the parent is applying too.
The care team
Records and letters when the application asks for them.
- Who decides
- NY State of Health decides which program the parent fits: Medicaid, the Essential Plan or a Marketplace plan with possible premium assistance.
- Ask the agency
- “Please test me, the parent, for Medicaid and the Essential Plan on this application. What is the income limit for my household this month?”
How to apply
First step: Apply for yourself on the same NY State of Health application as your child (nystateofhealth.ny.gov or 855-355-5777) and ask to be tested for Medicaid and the Essential Plan.
- Apply for yourself on the same NY State of Health application as your child.
- Report the month a paycheck stops: both programs use this month's income.

What to gather
- This month's income for the tax household
- Proof of any coverage loss (the end-date letter)
- Identity and New York address
How long: NY State of Health decides which program fits; Medicaid has a 45-day standard. Essential Plan coverage starts on the plan's start date after enrollment.
What a yes looks like
A notice naming Medicaid or the Essential Plan and a plan choice for the parent.
What a no looks like, and the next move
"Over 200%": then a Marketplace plan with possible premium assistance (up to 400% of the poverty line in 2026) is the parent's option. A drop in pay reopens this.
Watch out
- The Essential Plan's line is 200% of the poverty line since July 1, 2026, not 250%: pages printed earlier in 2026 say 250%.
- Both use this month's income. The month a paycheck stops is the month to report it.
- From January 1, 2027 some adults 19 to 64 face Medicaid work requirements. Under the federal rule a parent caring for a child 13 or under, or for a disabled person, is exempt; ask NY State of Health how it applies at your renewal.
Dates that change this
2026-07-01: The Essential Plan's upper limit reverted from 250% to 200% of the poverty line on July 1, 2026 (NYSOH notice of April 1, 2026). (not yet confirmed against the final rule)
2027-01-01: From January 1, 2027 some adults 19 to 64 have new Medicaid work requirements; a parent of a child 13 and under or of a disabled individual is exempt under the federal rule.
2025-12-31: The expanded premium tax credits ended December 31, 2025; no credit above 400% of the poverty line in 2026.
