A parent’s coverage matters during a child’s treatment too. North Carolina’s adult Medicaid route uses the parent’s household and current income. The county checks the adults separately from the child.
Eligibility rules
- The expansion group covers North Carolina adults aged 19 to 64.
- From April 2026, base monthly amounts are $2,399 for two, $3,028 for three, $3,658 for four and $4,288 for five.
- If your income is just over the line, the county adds an allowance before deciding (about $114 a month for three people, more for a bigger family), so a family slightly over the base amount can still qualify.
What you get
- No premium for adult Medicaid.
- Doctor visits, hospital care, mental health care and prescriptions.
What the help covers
- This coverage pays for the parent’s care. Your child has a separate eligibility decision.
If you decide to apply
- Ask the hospital social worker to include interested adults on the ePASS application.
- Have current income and each adult’s information ready for the county.

Good to know
A child’s approval does not automatically cover a parent. The same form can include both.
Other details
- From January 2027, some adults will need 80 hours a month of work or another activity, or earnings of about $580 a month, to keep this coverage. A parent caring for a child under 14, or for a disabled family member, is exempt.
- The scheduled January 2027 alternative is $580 monthly in qualifying income. The county checks which activities or income qualify and whether an exception applies.

- If a parent’s hours dropped, report this month’s income, not last year’s.

Dates that change this
2027-01-01: From January 1, 2027 North Carolina says adults 19 to 64 must work, study or volunteer 80 hours a month. The published exemptions include people with a disability and parents of children under 14.
