SoonerCare covers children under 19 whose income meets Oklahoma’s limit. Work insurance does not rule it out. The state decides who belongs in your child’s household and which income counts.
Eligibility rules
- Your child must live in Oklahoma and be under 19. The same income line applies to every age.
- Published monthly child limits effective April 1, 2026, for household sizes one through eight are $2,806, $3,806, $4,804, $5,802, $6,802, $7,800, $8,798 and $9,798. OHCA decides the child’s household and countable income; these figures already include the disregard.
- The dollar limits already include the five-percentage-point disregard. It is not added again.
- The state uses the child’s MAGI household. A yearly income divided by 12 is only an estimate of current monthly income.
- Full SoonerCare requires the child to meet citizenship or an eligible immigration category and the other program rules. Waiting periods and proof exceptions differ, so a qualified enrollment or legal adviser can check the child’s documents and the rule for the service date. Emergency-only SoonerCare is narrower than full coverage and does not automatically cover an entire leukemia treatment course.
How this can help alongside work insurance
- SoonerCare can be added without giving up the work plan. The work insurer pays first.
- The financial counselor can check how both plans cover your child’s hospital, medicines and treatment.
What to weigh
- Keeping both means the work-plan premium and two sets of billing rules.
- Insure Oklahoma is a different program. An applicant already enrolled in Medicaid cannot use that premium subsidy.

- No premium or child copays for covered hospital care, chemotherapy, clinic visits and medicines.
- Dental and vision care.
- Rides to covered appointments and home nursing when separately approved.
- Eligible bills from up to three months before the application can be covered through 2026.
What this covers
- Home nursing and personal care need their own assessment and approval.
- For children under 21, EPSDT can cover medically necessary care within federal Medicaid benefit categories. It does not cover every requested service.
- Care that neither plan covers, or a clinic not enrolled with SoonerCare, can still bill you. Ask billing to check any unexpected charge before you pay it.

- Have your child’s ID, Social Security number, Oklahoma address, current income, insurance cards and earlier bills ready.

What happens next
- The state decides eligibility. Most children then receive care through Aetna Better Health, Humana Healthy Horizons or Oklahoma Complete Health.
- Some online cases receive a quick result, but federal rules generally allow no more than 45 days for a non-disability application and 90 days for a disability application, apart from documented unusual circumstances. The social worker can help track a delayed case.
- Once approved, your child keeps SoonerCare for 12 months even if your income goes up. It ends sooner only if your child turns 19 or you move out of Oklahoma.
- For applications in 2026, covered bills may qualify for the three preceding calendar months if your child met the rules in each month. Federal law schedules a two-month child look-back for applications from January 1, 2027. The counselor can confirm Oklahoma’s implementation and the dates for your bills.
- The social worker can ask whether this hospital can start temporary hospital presumptive coverage. An authorized employee of a participating hospital must approve it. A full application filed by the end of the following month keeps that temporary coverage running until the decision. Without that application, coverage normally ends at the end of the following month. The separate hospital notice-of-date-of-service process does not create temporary coverage or a decision deadline.
Good to know
Your child can have both plans. The work plan is billed first, so the hospital needs both cards.
Other details
- Before choosing a health plan, the hospital can check its oncology group, pharmacy and planned treatment.
- Keeping work insurance means its premium and two insurers to manage. Removing a child may not reduce a family premium.
- If your income is over the children's line, TEFRA (next card) looks only at the child's own finances and care needs.

Who does what

A notice with coverage dates and a health-plan choice. The hospital can check oncology participation. Children generally keep 12 months of coverage, with limited exceptions.
What a no looks like, and the next move
Read whether the no was about income, a document or a category. If it was income, ask for TEFRA by name and appeal by the date on the notice.
Watch out
- Oklahoma uses one income column for every age, so a teenager has the same limit as a baby.
- The printed dollars already include the five-point allowance; do not add it again.
- Your child can keep private insurance and have SoonerCare too. Do not cancel the work plan to apply.
- If the answer is no on income, ask in the same breath about TEFRA, which leaves the parents' income out.
Dates that change this
2026-04-01: The income limits are the table effective April 1, 2026. No fixed annual adoption date is published, so do not assume an April change every year.
2027-01-01: The agency has circulated a proposal to cut back coverage from three months to two for applications made on or after January 1, 2027. It is a proposal, not an approved change. (not yet confirmed against the final rule)
2026-10-01: A proposed rule on noncitizen eligibility has been published with a requested date of October 1, 2026, contingent on federal approval. Ask the agency how it applies to your child. (not yet confirmed against the final rule)

The trap: Oklahoma prints ONE income column for every age, so a teenager has the same limit as a baby; do not assume the limit drops as the child gets older. The state says the dollar figures already include the five-point disregard, so do not add five points yourself. Do not cancel private insurance to apply.
