What it is

A parking placard that follows an eligible child, including when riding with another adult.

A shorter walk from the car can matter when a child has mobility limitations. A clinician must support the disability placard application. Treatment alone does not establish that the child meets the parking criteria.

Eligibility rules
  • An eligible child can use the placard as a passenger.
  • Accepted signers include a physician, physician assistant, advanced registered nurse practitioner, osteopathic physician, chiropractor, podiatrist or optometrist.
  • The signer must certify a listed medical criterion, such as inability to walk 200 feet without resting, needing assistance to walk, a qualifying serious lung or heart condition, portable oxygen, specified severe walking limitations, legal blindness or qualifying limb loss. Cancer or treatment alone is not a criterion. Service Oklahoma currently links form 302DC revised January 2023; confirm the certification and submission date.
