A Medicaid route where medical bills can help a family qualify when income is too high.
A spend-down may help when ordinary Medicaid is out of reach. Vermont Health Connect sets the amount of medical bills needed for a six-month period. Bills you already owe can count.
Eligibility rules
- Vermont includes people under 21 among the groups that can use this route.
- This route requires a qualifying medically needy category, including eligible children under 21 and certain parents or caretaker relatives. The worker compares income that counts with the protected amount for that category and period. The Medicaid for Children and Adults medically needy route has no resource test; disability-based categories use different rules. Vermont Health Connect confirms the current protected-income amount.
What you get
- Medicaid coverage from the day qualifying bills meet the spend-down through the end of the approved period, if the other rules are met.
What the help includes
- Older unpaid bills can contribute to meeting the amount. A bill used to meet it is different from a bill Medicaid will pay.
- The worker can count allowed incurred expenses after checking dates, other insurance and whether the bill was used before. Bills assigned to meet your spend-down remain your responsibility; Medicaid does not simply pay them back. Coverage can start on the day the amount is met and run to the end of the approved six-month period. The notice should show both the bills you still owe and the coverage dates.

- Ask hospital billing for itemized bills, including earlier unpaid bills, and bring them to the worker.

After you ask
- The worker checks which bills count and when the required amount is met.
- The usual Medicaid decision standard is 45 days, or 90 days for a disability determination, with limited exceptions. The worker separately decides when your bills meet the spend-down. A new six-month period needs an updated calculation; the notice should explain what must be renewed or submitted and which old bills can still count.
Good to know
A spend-down needs a separate request. The worker sets both the amount and the six-month dates.

- A large admission may reach the amount, but the bill dates and the chosen period matter.

Watch out
- Nothing happens unless you ask. The state page says you have to ask for a spend-down and be found eligible.
- Vermont counts six months at a time, so one admission can carry the whole period. Ask for the exact dates in writing.
- Vermont has never published the dollar standards, so no figure here is a promise. The worker sets yours.
