How this works
Virginia’s spend-down route uses qualifying medical bills to reduce the income used in its Medicaid test.
The ordinary children’s group is for children under 18. Parents’ income counts at home.
The agency uses its published six-month locality standard; monthly figures cannot simply be multiplied.
Savings limits are $2,000 for one person, $3,000 for two, plus $100 per additional person.
- For an eligible child, coverage can begin when the spend-down is met, followed by 12 months of continuous coverage, subject to the children’s coverage exceptions.
- Expenses used to meet spend-down can remain your responsibility. Old unpaid bills can count only if not already used for an earlier spend-down.
- Ask the hospital social worker to review the Virginia program with you.

- The standard is far below the ordinary Medicaid line. Most families need the disability options first.
