B-ALL is in scope. A child with B-ALL can fit the critically ill or medically complex scope when the separate ICU and extended-hospitalization conditions are met.
Age
Age 0 to age 18, measured at application.
- The published maximum is age 18 or younger.
Treatment status
Active Treatment, Relapse Or Recurrence, Palliative Or Hospice
- The child must be in or expected to be in ICU and expected to remain hospitalized for at least one week or one more week from referral.
- The family must live at least 30 miles from the treating hospital.
- A hospital, physician-office, or Ronald McDonald House professional must submit the referral.

- Ask the hospital case manager, social worker, nurse, physician office, or Ronald McDonald House staff to submit the referral.
- Prepare and submit the travel referral with family verbal consent and the published eligibility facts.
- Verify eligibility and tell the family whether to complete the two follow-up authorization forms.
- After eligibility is verified, complete the health-information and likeness authorization forms.
