How to start
Fill in the Travel Assistance form with your child's care provider, then scan, fax or mail it with your receipts.

Cancer Connection pays back up to $750 each calendar year toward airfare, ferry, lodging and ground transportation when a cancer patient from a Southeast Alaska community travels outside that community for cancer care. The patient (or family) and a health care provider complete a one-page patient data form and send it by scan, fax or mail, with receipts. No income information is asked.
