Icing Smiles provides free custom Dream or Fun Cakes to medically qualified children ages 1–18 and their siblings, plus Memorial Cakes for families after a child's death. Families pick up the cake from an assigned volunteer baker. A parent or guardian applies online and, for medical eligibility, obtains a specialist-physician form or accepted partner document.

Diagnosis
B-ALL is in scope. The 2026 official eligibility sheet expressly lists ALL under Oncology.
Age
Age 1 to age 18, measured at service date.
- The event date must be within three months of turning 18; sibling eligibility also ends after the medical child turns 18.
Treatment status
Active Treatment, Within One Year Post Treatment, Post Treatment, Palliative Or Hospice, Bereavement
- Oncology eligibility generally extends through two years after completion of chemotherapy and/or radiation.
- An alternate hospitalization pathway covers 30 or more hospital days or more than six overnight hospitalizations in the last 12 months.
- Memorial Cakes cover losses within the past two years.
- A specialist physician completes and wet-signs the Medical Eligibility Form unless an accepted partner form or Make-A-Wish acceptance letter is submitted.
- Cakes are free regardless of financial need.

How to get started
Does a social worker or care team need to start it? No. A required care-team start is not published; the family can use the available route below.
Who can start: Parent Or Guardian
- Complete the online cake application and select medical child, sibling, or memorial cake.
- Give the current MEF to the child's specialist physician, unless uploading an accepted partner document.
- Complete every required MEF field, apply the office stamp or patient label, wet-sign, and return it directly to Icing Smiles.
- Confirm both the confirmation page and receipt email; contact Icing Smiles if either is missing.
