- Download the Physician Consent Form or ask MATIO to send a blank copy to the physician office.
- Document diagnosis, relevant treatments, allergies or conditions, and participation clearance or restrictions; sign the form.
- Upload the signed physician form during registration, or arrange for the physician office to email or fax it.
