You can refer your child to A Special Wish Foundation from birth through age 20 if they have a life-threatening or terminal illness, live in Ohio or the Chicago area, and have not received another organization's wish. Anyone can make the referral. The form asks for your family's contact details, your child's diagnosis, physician and social worker, and the referrer's information. The foundation reviews the referral and handles the next steps.
