The patient must have been diagnosed on or before age 21 and treated before age 24, the family must demonstrate need, and easily accessible bank balances must be below $5,000. A parent, guardian, or social worker completes the online form, supplies social-worker information and proof of diagnosis, and receives a status response target of 48 hours.
