Montana Children’s Special Health Services may provide up to $2,000 per year for approved treatment, enabling services, or items for a Montana resident from birth through age 21 with a qualifying chronic condition and extra healthcare needs. A child with B-ALL fits the broad chronic-physical-condition rule when the provider documents medically necessary treatment or an item related to the diagnosis, but other funding sources must be exhausted first and household income may not exceed 300% of the federal poverty level. A parent or guardian and licensed Montana medical provider complete the application, attach tax, insurance, and other-resource records, and submit it by email or fax.
