The patient must be 22 or younger and in treatment, household income must be at or below 350% of the federal poverty guideline, and an authorized healthcare professional submits the application and letterhead attestation. Project Smile provides toys, crafts, gift cards, parking passes, and similar support during extended admissions or long clinic days through participating hospitals, with UMass Memorial Children's Medical Center named as the most recent partner.
