Before you start, check you fit

  • Financial or another transportation barrier must be verified by a third party.
  • The passenger must be medically stable, ambulatory, and able to sit upright with a seatbelt.
  • The service is not an air ambulance and does not provide medical assistance.

    Angel Flight Southeast needs a signed release from your doctor. Copy this message, or say it in person at the next visit.

    • Start with the official request form or call the Mission Request Hotline during published hours.
    • The physician returns the completed signed medical release directly to Angel Flight before the scheduled mission.

After you send

Confirm receipt of the medical release and retain a backup travel plan until the mission is confirmed. If you have not heard back in two weeks, write and ask whether it arrived.
