Before you start, check you fit

  • Your child is 21 or under.
  • A prior dream wish from another organization rules out a new Dream request.
  • No reimbursement for purchases; phone photos and faxed forms are not accepted.

    What to bring to the meeting

    • Form 3 Parent/Guardian Questionnaire.
    • Form 4 release signed by parent/guardian, patient if age 18 or older, additional participants, and two adult witnesses.

    What happens next

    • Confirm the eligible referrer and whether the request is under or over $500.
    • If the request is over $500, the treating-hospital social worker or doctor completes Form 1.
    • If the request is over $500, the attending physician completes and signs Form 2.
    • If the request is under $500, complete Form 5 and obtain all parent, patient-if-applicable, and witness signatures.
