Does a social worker or care team need to start it? Yes. Ask your child’s social worker or care team to begin the request.
Who can start: Social Worker, Healthcare Team, Physician, Community Referrer
- Confirm the applicant meets the published referral, serious-illness, age, prior-wish, and request-location rules.
- For requests over $500, complete Form 1 and coordinate the attending physician’s Form 2.
- For requests over $500, complete Form 3 and all applicable Form 4 releases; for requests under $500, complete Form 5 with the eligible referrer.
- Return complete, legible forms with all required signatures by email or mail; do not fax and do not submit phone photos.
