    Have ready

    • Mileage reimbursement form for a prior-authorized personal-vehicle trip, due within 60 days after the appointment.

    • Schedule the trip through MAS online or by contact center before travel.
    • The medical provider electronically submits any required 2015 or 2020 form.
    • Submit mileage reimbursement documentation within 60 days after an approved personal-vehicle appointment.

After you send

Confirm authorization and pickup details before the appointment; use the MAS contact center for changes or urgent/discharge requests. If you have not heard back in two weeks, write and ask whether it arrived.
