Before you start, check you fit

  • Trips require prior authorization and advance notice except where current rules provide an exception.
  • Plan, covered service, medical necessity, ride availability, notice, attendant, lodging, meal, and out-of-state authorization rules apply.

    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • The MCO verifies the covered service and provider and completes all sections of Form 4214, including any required long-distance justification.
    • The MCO submits the completed form to the assigned transportation organization or broker for authorization.

After your care team sends it

The member or family follows up with the transportation organization, broker, or MCO about authorization and trip arrangements.
