  1. Complete the family portion of the Emergency Financial Assistance application and have a parent or guardian sign it.
  2. Contact the child's hospital social worker and ask about Candlelighters Emergency Financial Assistance.
  3. Have the parent or guardian sign the New Family Form supplied by the social worker.
  4. If approved, Candlelighters pays the institution or business directly.
  5. If the child does not have a hospital social worker, contact Nicole at Candlelighters by phone or email.
  6. Review the current official program page and use the published family-services, social-worker, registration, or on-site route.
  7. The hospital social worker or other hospital medical staff person submits the request on the family's behalf.
