Diagnosis
B-ALL is in scope. A child with B-ALL is eligible when treatment causes medically related hair loss and the other published requirements are met.
Age
Age 0 to age 20, measured at application.
- Applicant must be under age 21.
Treatment status
Active Treatment, Maintenance Treatment, Relapse Or Recurrence, Post Treatment, Survivorship
- Current treatment is not required if medically related hair loss persists; chemotherapy applicants must include the most recent treatment date in the doctor letter.
- A doctor must provide a signed and dated letter on letterhead documenting the required medical facts.
- No income test is published.

- Gather identity documents, before/current photos, and head circumference measurement.
- Ask the doctor for the required signed and dated letter on letterhead.
- Watch the official hair-care videos with everyone who will care for the replacement.
- Complete the online form, upload all documents and photos, and submit.
- For missing documents or extra photos after submission, email the program and include the applicant's name.
