A parent or guardian can apply online for a child or young adult under 21 who lives in the United States, is a U.S. citizen or permanent resident, and has medically related hair loss. Prepare identity documents, a signed and dated doctor letter on letterhead, before-and-current hair-loss photos, measurements, and the online answers before starting.
