What you get

Up to $2,000 per 12-month grant for counseling, talk or cognitive behavioral therapy, covered medicine copays, telehealth, magnetic-stimulation treatment, travel and parking for behavioral health care.

Call HealthWell to apply. After pre-approval, ask your Medicare-certified behavioral health clinician to sign and send the fund statement through the provider portal or by fax.

• A provider must verify the patient or diagnosis.
• A Medicare-certified psychiatrist, psychologist, licensed social worker, or other covered behavioral health clinician must prescribe or provide the covered service.
• After pre-approval, the treating clinician must confirm the caregiver's behavioral health diagnosis, caregiving relationship, current treatment, and provider certification on the fund statement.
• The caregiver must have private insurance, Medicare, Medicaid, TRICARE, or another form of insurance that covers part of the behavioral health treatment or medication.

Financial need is required.

Does a social worker or care team need to start it? No. A required care-team start is not published; the family can use the available route below.

Who can start: Caregiver

1. After pre-approval, have the treating Medicare-certified behavioral health clinician complete and sign the OCBH Fund Statement and submit it through the Provider Portal or by fax.
2. Call HealthWell to apply while the caregiver fund is open.
3. Confirm that the oncology patient has an active HealthWell oncology grant and that the caregiver lives with and provides primary care to that patient.
4. Provide insurance, household-income, Social Security number, relationship, shared-residency, and behavioral-health treatment information requested by HealthWell.
5. Submit shared-residency documents if HealthWell requires the Residency Verification form.

Documents to prepare

• Completed and clinician-signed Oncology Caregiver Behavioral Health Fund Statement after pre-approval
• HealthWell identification or underlying active oncology-grant information
• Insurance and household-income information
• Proof of shared household residency and immediate family relationship if requested, such as tax return, lease, identification, insurance, mortgage, bank, utility, voter-registration, or vehicle document
• Social Security number for grant creation

• HealthWell may use a pharmacy card, direct billing, or reimbursement for eligible cost shares.
• Ride Health trips should be requested at least two business days before the eligible appointment and are deducted from the grant.

Availability and timing

• For pediatric B-ALL matching, the underlying Pediatric Assistance Fund is closed to new patients due to insufficient funding, but an existing pediatric grant remains active for its 12-month cycle or until exhausted.
• The official caregiver fund page states Open — Accepting phone applications only.
• The fund is open for phone applications as of August 24, 2026, but may close or change when available resources are exhausted.
• Call at least two business days before an eligible appointment when requesting Ride Health transportation.
• The fact sheet says phone applicants receive instant notification of an initial approval decision, but subsequent clinical and residency verification may still be required.
• Initial phone approval notification may be immediate; final grant use depends on required verification and claims processing.

Limitations and things to confirm

• A caregiver cannot qualify solely because a child has B-ALL; the oncology patient must already have an active HealthWell oncology grant.
• Application requires sensitive information including a Social Security number, behavioral health diagnosis, family relationship, and potentially shared-residency evidence; HealthWell says the Social Security number is kept secure and confidential.
• HealthWell's B-ALL-covering Pediatric Assistance Fund is closed to new patients as of August 24, 2026, so this pathway is currently limited to existing active pediatric grant holders.
• Household income must meet the published 500% FPL guideline and assistance is limited to $2,000 per 12-month grant.
• Ride Health requests require at least two business days' notice, and transportation fees are deducted from the grant.
• The caregiver must be an insured, live-in immediate family member and primary caregiver receiving prescribed behavioral health treatment from a Medicare-certified provider.
