New York law requires licensed hospitals to offer a uniform financial-assistance process for emergency and medically necessary hospital care, including pediatric cancer care. Published statewide rules cover households up to 400% of the federal poverty level when the patient is uninsured, has exhausted benefits, or is underinsured with paid medical expenses over 10% of gross annual income; immigration status cannot be used as an eligibility criterion. Families submit the uniform form to the treating hospital’s financial-assistance office, which must decide a complete application within 30 days, while provider coverage and non-emergency service-area rules can vary by hospital. New York’s Children’s Waiver can authorize home- and community-based services for a Medicaid-enrolled or Medicaid-eligible child under 21 who meets the published functional and institutional-risk standards; the medically fragile pathway explicitly includes cancer. Available plan-of-care services can include respite, caregiver and family advocacy, palliative-care services, habilitation, and other assessed supports. A family without Medicaid contacts C-YES for evaluation, while a child already enrolled in Medicaid starts through a Health Home; a B-ALL diagnosis by itself does not establish the required disability and level-of-care findings. New York Medicaid Non-Emergency Medical Transportation arranges prior-authorized travel to and from Medicaid-covered medical services at no cost to the enrolled member. Available modes include public transit, taxi or livery, ambulette, ambulance, and personal-vehicle mileage reimbursement, with the most medically appropriate and cost-effective mode used. Families schedule through Medical Answering Services before the appointment; travel outside the Common Medical Market Area and specialized vehicle modes require provider forms, and mileage claims must be submitted within 60 days.
